Prior to moving to campus, the university requires that all residents provide proof or a negative tuberculosis test or chest x-ray ON A ANNUAL BASIS (NEW university policy). The Tuberculosis Verification Form (TB) must be submitted (to the Health Center) by ALL students wanting to live on campus.
The Meningitis Form is required from all first-year students moving into Housing. Please review the information and check off if you will or will not receive the vaccination and submit it to the Housing Services Office.
If a resident is under the age of 18, a parent or guardian must sign and agree to the terms and conditions of the License Agreement and complete the Consent Authorization Form for Medical Treatment.
