This form should be used to request the installation Office 2011 for Mac on any Mac OS X 10.5.5 or greater campus computer
- Requests with missing information may delay the installation of software.
- The software will be installed on your baseline provided computer no later than five business days.
- Fill out and submit the form below.
- Upon submission, you will receive a confirmation e-mail.
- You may be contacted for further information or instructions.
- Use the following to find out your Computer Name:
- Mac - Click Apple to System Preferences then select Sharing. The computer name appears as "Computer Name" on that page.
- Mac - Click Apple to System Preferences then select Network. The IP address appears in the middle box.
